Vaccines are the safer option when confronted with a choice between leaving a child vulnerable to disease or vaccinating them.
The argument
Proponents of this position begin with an observation about where doubt is concentrated. The fiercest sceptics of vaccine safety are from the higher-income countries of Western Europe and North America. This is because these populations do not have to face the deadly consequences of the infectious diseases that ravage places like Bangladesh and Rwanda, where there is almost unanimous support and trust of vaccinations. Where a disease is a present danger, the comparison people make is between the vaccine and the illness; where the illness has been absent for generations, it drops out of the comparison altogether and only the vaccine is left to be weighed. That, on this argument, is why the question is framed as it is. The only reason the safety of vaccines is widely discussed is because people in the West have not directly experienced the horrors of whooping cough or polio. Once the missing half of the comparison is restored, the claim being made is not that vaccines carry no risk at all, but that vaccines, even if they are not perfect, are far safer than leaving children exposed to these diseases. Safe, in this context, is a relative term: it describes which of two available risks is the smaller one, not the absence of risk. The numbers are what make that comparison decisive. The risk of a very serious reaction to a vaccine is extremely rare — 1 in 1 million doses, according to the CDC — and seizures after vaccines are also very rare. In contrast, the risk of a child with measles contracting pneumonia is 1 in 20. Around 1 in every 1000 children with measles may develop encephalitis, a swelling of the brain that can cause deafness or intellectual disability, and for around 1 or 2 of every 1000 children, measles is even fatal. Set side by side, the two sets of odds differ by orders of magnitude, and each of the measles outcomes is a consequence of declining the vaccine rather than of accepting it. Judged on that comparison, vaccines are safe.

For accurate information on the efficiency of vaccines, consult the WHO website. Among communities that face infectious diseases on a daily basis, where the benefits of vaccines are most visible, there are fewer fears over vaccine safety. This is because, relatively speaking, vaccines are "safe". The risks of not being vaccinated far outweigh the risks of being vaccinated. When we deal with medication and treatment, relative safety should be the only thing that matters.

Critics reply that comparing vaccine risk to disease risk answers a different question than "are vaccines safe." A favourable risk-benefit ratio establishes that vaccinating is the wiser choice, not that vaccines are categorically "safe"; recasting safety as merely relative sidesteps the demand — which many raise in good faith — for rigorous monitoring of rare but real adverse events. They also object to the framing that scepticism is just Western affluence. Whether a doubt is well-founded does not depend on the wealth of the person raising it, and the headline "1 in 1 million" is an aggregate that varies by vaccine and recipient. Dismissing safety questions as complacency risks discouraging exactly the surveillance that keeps vaccines safe.
